Contract

1.- This child care home is committed to providing a safe environment where your child will be happy and enjoy learning in a family environment
2.- The child care home will be open from 8:00 am to 5:00 pm ( if your child arrives before or after the above hours your fee will be different.
3.- Breakfast is served from 8:00 am to 8:30 am. Lunch is served from 11:30am to 12:00pm Snack is served after nap time.
4.- I will notify the child care home of my child absence before 9:00am
5.- I will notify the child care home of changes in my address or telephone number (s) 6.- Children should have all vaccinations appropriate for their age as required by State of CA 7.- My child will no be allowed in child care if He/She has any of the following symptoms: Severe Cold, Fever, Green Mucous , Pink eye , Diarrhea.
8.- If my child becomes ill during child care hours, I will be called and need to pick up as soon as possible.
9.- I give my permission to Georgina Manzo to administer to my child any doctor prescribed medicine, including medicine for pain and fever (aspirin/ibuprofen)
10.- Anybody who picks up my child will be on the emergency list and will present their photo ID upon pick up
11.- My child will not be releases to someone that is suspected to be under the influence of alcohol or drugs.
12.- I agree to pay a nonrefundable deposit in the amount of _________. 13.- The child care FEES is based on the child care hours established for each parent. 14.- IM REQUIRED TO PAY A DAILY CHILD CARE FEE FOR ALL DAYS INCLUDING MY FAMILY VACATION, HOLIDAYS, FAMILY EMERGENCIES, ILLNESSES AND TWO WEEKS OF VACATION FOR GEORGINA MANZO. NEW YEAR’S EVE AND NEW YEAR’S DAY, PRESIDENT’S DAY, MEMORIAL DAY, INDEPENDENCE DAY, LABOR DAY, THANKSGIVING AND THE DAY AFTER , AND CHRISTMAS EVE AND CHRISTMAS DAY.
15.- I need to give two weeks notice before my child last day or PAY FOR TWO WEEKS 16.- All payments are due on MONDAY before start the week ,unless there is holiday, the payments are due the following day . if payments are not received by Monday there is a $ 35.00 weekly late fee
17.- Please make checks payable to GEORGINA MANZO . any returned checks will be subject to a $ 35.00 processing fee.
18.- If my child is picked up after established hours I will be charged $ 2.00 for every minute I am late ( This fee is Payable immediately at pick up and cash)
19.- Georgina Manzo may terminate this contract at any time. ( If you break this contract).
